Изучить связь уровней подоцина с атеросклеротическим поражением артерий у пациентов с СД 2 типа.Материалы и методы. В исследование были включены 316 пациентов с СД 2 типа, медиана возраста 66,00 [60,00; 72,00] лет. Проводилось стандартное обследование с оценкой уровней подоцина в моче и ультразвуковое исследование брахиоцефальных артерий с определением толщины комплекса интима-медиа (КИМ).Результаты. Уровни подоцина у пациентов с СД 2 типа были достоверно выше по сравнению с группой контроля (0,615 [0,320; 1,892] против 0,240 [0,140; 0,300] нг/мл, p &lt; 0,001). При проведении корреляционного анализа взаимосвязи подоцина и КИМ ОСА нами была установлена заметной тесноты прямая связь (r = 0,524 для КИМ справа и r = 0,518 для КИМ слева при p &lt; 0,001). Методом бинарной логистической регрессии была определена достоверная связь подоцина с КИМ ОСА, так при увеличении подоцина на 1 нг/мл шансы утолщения КИМ увеличивались в 3,028 раза. Уровень подоцина ≥ 0,806 нг/мл ассоциировался с вероятностью утолщения КИМ с чувствительностью и специфичностью 71,0% и 81,0%, соответственно (AUC ROC 0,778 ± 0,047 с 95% ДИ: 0,686 – 0,871, p &lt; 0,001).Заключение. Подоцин в моче может рассматриваться в качестве раннего маркера атеросклероза у пациентов с СД 2 типа, с чувствительностью 71% и специфичностью 81%.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Diabetic nephropathy, on the one hand, is one of the most common causes of end-stage chronic kidney disease in diabetes mellitus (DM), and on the other hand, a risk factor for the development of cardiovascular complications. Microalbumin has traditionally been used in routine practice to assess renal function and cardiovascular risk, however, many markers are now being identified that are associated with renal dysfunction, one of which is podocin. In this regard, it becomes relevant to study podocin in order to predict the risk of developing cardiovascular diseases in diabetes in the future.Purpose. To study the relationship between podocin levels and atherosclerotic arterial lesions in patients with type 2 diabetes.Materials and methods. The study included 316 patients with type 2 diabetes, median age 66.00 [60.00; 72.00] years. The investigation of patients included a standard examination with assessment of podocin levels in urine and ultrasound examination of the brachiocephalic arteries with determination of intima-media thickness (IMT) of the common carotid artery (CCA).Results. Podocin levels in patients with DT2 were significantly higher compared to the control group (0.615 [0.320; 1.892] vs. 0.240 [0.140; 0.300] ng/ml, p &lt; 0.001). When conducting a correlation analysis of the relationship between podocin and IMT CCA, we established a close direct relationship (r = 0.524 for IMT on the right and r = 0.518 for IMT on the left with p &lt; 0.001). Using the binary logistic regression method, a significant association of podocin with IMT CCA was determined, so with an increase in podocin by 1 ng/ml, the chances of IMT thickening increased by 3.028 times. When assessing the probability of IMT thickening using ROC analysis, a cut-off point for podocin of 0.806 ng/mL was obtained, with a sensitivity and pecificity of 71.0% and 81.0%, respectively. The area under the ROC curve was 0.778 ± 0.047 with 95% CI: 0.686 – 0.871, p &lt; 0.001.Conclusion.
