Изучить клинико-функциональные особенности состояния сердца и развернутого цитокинового профиля у госпитализированных пациентов с коронавирусной инфекцией COVID-19 и сопутствующей ИБС.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. Ретроспективное исследование включало анализ медицинской документации с определением основных характеристик пациентов, показателей летальности, оценкой полиморбидного фона, результатов аутопсий у 247 пациентов, госпитализированных с диагнозом COVID-19. В проспективное исследование включены 90 пациентов, проходящих стационарное лечение в кардиологическом отделении с диагнозом COVID-19. Пациенты были разделены на 2 группы: группа 1 представлена пациентами с COVID-19 и ИБС (n = 42), группа 2 – пациентами с COVID-19 без ИБС (n = 48). Обследование пациентов включало в себя определение стандартных биохимических показателей, оценку цитокинового профиля (48 цитокинов и хемокинов), эхокардиографическое исследование (ЭХО-КГ), компьютерную томографию легких (КТ).</p></sec><sec><title>   Результаты</title><p>   Результаты. При исследовании аутопсийного материала сердечно-сосудистой системы гипертрофия левого желудочка была выявлена у 185 человек (74,9 %). По результатам проведенных исследований в группе 1 наблюдались наибольшие значения воспалительных цитокинов, таких как интерлейкин (IL): IL-1, IL-6, колониестимулирующий фактор гранулоцитов(G-CSF), моноцитарный хемотаксический протеин-1 (MCP-1), интерферон-гамма индуцируемый протеин-10(IP-10), воспалительный белок макрофагов 1b(MIP-1b), фактор некроза опухоли-а (TNF-а) относительно группы 2. Выявлена положительная связь между уровнем С-реактивного белка и IL-2,IL-6,IL-7, IL-17a, G-CSF, MCP-1, IP-10, воспалительный белок макрофагов-1а (MIP-1a), MIP-1b, TNF-a, ферритина и IL-2, IL-6,IL-17a, G-CSF, MCP-1, IP-10, MIP1a, MIP-1b, TNF-a, а также между уровнем D-димера и IL-2, IL-6,IL-7,IL-17a, MIP-1a, MIP-1b, TNF-a. По данным ЭХО-КГ в группе 1 выявлено статистически значимое увеличение продольного размера правого предсердия, уровня среднего давления в легочной артерии(CДЛА), расширение поперечного размера левого предсердия по сравнению с группой 2 (р &lt; 0,05).</p></sec><sec><title>   Заключение</title><p>   Заключение. На основании полученных результатов о выраженном системном воспалении, которое развивается у пациентов с коронавирусной инфекцией с сопутствующей ИБС, следствием которого являются быстроразвивающиеся морфо-функциональные изменения миокарда, можно рассматривать коронавирусную инфекцию как фактор, усугубляющий дальнейшее течение ИБС.</p></sec></abstract><trans-abstract xml:lang="en"><p>   The analysis of cytokine profile markers with reference to the clinical and functional features of the heart condition in case of new coronavirus infection (COVID-19) can provide valuable information about the pathogenetic mechanisms of cardiovascular complications and be considered as an additional factor of unfavorable prognosis in patients with concomitant coronary heart disease (CHD).</p><sec><title>   Aim</title><p>   Aim. To study the clinical and functional features of the heart condition and the expanded cytokine profile in hospitalized patients with COVID-19 coronavirus infection and concomitant coronary artery disease.</p></sec><sec><title>   Material and methods</title><p>   Material and methods. The retrospective study included an analysis of medical documentation to determine the main characteristics of patients, mortality rates, assessment of the polymorbid background, and autopsy results in 247 patients hospitalized with COVID-19. The prospective study included 90 patients undergoing inpatient treatment in the cardiology department with a diagnosis of COVID-19. The patients were divided into 2 groups: group 1 represented by patients with COVID-19 and coronary heart disease (n = 42), group 2 – patients with COVID-19 without coronary heart disease (n = 48). The examination of patients included the determination of standard biochemical parameters, assessment of the cytokine profile (48 cytokines and chemokines), echocardiography (ECHO), computed tomography of the lungs (CT).</p></sec><sec><title>   Results</title><p>   Results. In the study of autopsy material of the cardiovascular system, left ventricular hypertrophy was detected in 185 patients (74.9 %). According to the results of the conducted studies, the highest values of inflammatory cytokinesinterleukins(IL): IL-1, IL-6,granulocyte colony-stimulating factor (G-CSF), monocyte chemoattractant protein-1(MCP-1),interferon γ-induced protein-10 (IP-10), macrophage inflammatory protein-1b(MIP-1b), tumor necrosis factor-a(TNF-a) were observed in group 1 relative to group 2. A positive relationship was revealed between the level of C-reactive protein and IL-2, IL-6, IL-7,IL-17a, G-CSF, MCP-1, IP-10, macrophage inflammatory protein-1a(MIP1a), MIP1b, TNFa, ferritin and IL-2, IL-6, IL-17a, G-CSF, MCP-1, IP-10, MIP1a, MIP1b, TNFa, as well as between the D-dimer level and IL-2, IL-6, IL-7, IL-17a, MIP1a, MIP1b, TNF-a. According to ECHO data, a statistically significant increase in the longitudinal size of the right atrium, the level ofmeanpulmonary artery pressure (mPAP), and an expansion of the transverse size of the left atrium were revealed in group 1 compared with group 2 (p &lt; 0.05).</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Based on the findings obtained regarding pronounced systemic inflammation that develops in patients with coronavirus infection associated with concomitant coronary heart disease, it resulting in rapidly developing morpho-functional changes in the myocardium, coronavirus infection can be considered as a factor exacerbating the further course of coronary heart disease.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>ишемическая болезнь сердца</kwd><kwd>короновирусная инфекция</kwd><kwd>сосудистое старение</kwd><kwd>экспрессия генов</kwd><kwd>сердечно-сосудистые заболевания</kwd></kwd-group><kwd-group xml:lang="en"><kwd>coronary heart disease</kwd><kwd>coronavirus infection</kwd><kwd>vascular aging</kwd><kwd>gene expression</kwd><kwd>cardiovascular diseases</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Shi S., Qin M., Shen B., Cai Y., Liu T., Yang F. et al.
