Формирование СН у пациентов с ОИМ значительно ухудшает прогноз и увеличивает затраты на лечение, что определяет медико-социальную значимость проблемы. Это подчеркивает необходимость не только тщательного мониторинга за пациентами с ОИМ, но является обоснованной мотивацией использования мультимодального подхода к определению индивидуального риска развития сердечной недостаточности у пациентов с ОИМ, поскольку в развитии СН участвуют разные патогенетические механизмы с различным вкладом ведущих нарушений: микроциркуляторной дисфункции, воспаления, кровоизлияния, отека, ремоделирования. Практической реализацией мультимодального подхода является определение клинических, лабораторных, ангиографических и визуализирующих предикторов формирования СН. Для стратификации риска необходимо обосновать оптимальную комбинацию методов прогнозирования, чтобы определить степень риска формирования сердечной недостаточности, ключевой патогенетический механизм развития и, соответственно этому, использовать наиболее адекватные схемы лечения.</p></abstract><trans-abstract xml:lang="en"><p>Acute myocardial infarction (AMI) is a starting point for the development of heart failure (HF), which increases the risk of death. The development of HF in AMI patients significantly worsens the prognosis and increases treatment costs, which determines the medical and social significance of the problem. This not only emphasizes the necessity of careful monitoring of AMI patients, but also is a reasonable motivation for the use of multimodal approach to determine the individual risk of heart failure development in patients with AMI, since different pathogenetic mechanisms with different contribution of leading disorders are involved in HF development: microcirculatory dysfunction, inflammation, hemorrhage, edema, remodeling. Multimodal approach is implemented by the identification of clinical, laboratory, angiographic and imaging predictors of HF development.
