Для пациентов с ревматоидным артритом характерны ремоделирование левого желудочка с развитием гипертрофии его стенок и диастолической дисфункции, большая масштабность атеросклеротического поражения каротидного и коронарного бассейнов. Системное воспаление у пациентов с ревматоидным артритом способствует развитию большей выраженности процессов эндотелиальной и металлопротеиназной активации с увеличением содержания в крови растворимой межклеточной молекулы адгезии-1 и матриксной металлопротеиназы-9. Наиболее информативными признаками, детерминирующими степень атеросклеротического поражения коронарных и сонных сосудов у пациентов с ревматоидным артритом, являются:показатели воспалительной активности (уровни высокочувствительного С-реактивного белка, значение индекса DAS28), содержание мозгового натрийуретического пептида, возраст и наличие артериальной гипертензии.

Patients with rheumatoid arthritis are characterized by left ventricular remodeling with the development of its hypertrophy, diastolic dysfunction and a large-scale carotid and coronary atherosclerosis. Systemic inflammation in patients with rheumatoid arthritis promotes increases in blood levels of soluble intercellular adhesion molecule-1 and matrix metalloproteinase-9. The most informative features that determine the degree of coronary and carotid atherosclerosis in patients with rheumatoid arthritis are indicators of inflammatory activity (high-sen¬sitivity C-reactive protein levels, the index DAS 28), the content of brain natriuretic peptide, age and presence of hypertension.
