Методы: проведена оценка вариативности градиента давления между сокращениями у пациентов с выраженным аортальным стенозом и сопутствующей фибрилляцией предсердий на основе ретроспективного анализа допплеровских или катетерных гемодинамических измерений и изучена его корреляция с предшествующим интервалом RR на основании данных соответствующей электрокардиограммы. Результаты: существует прямая корреляция между предшествующим интервалом RR и средним градиентом давления у пациентов с выраженным аортальным стенозом и сопутствующей фибрилляцией предсердий. Эта вариативность, по-видимому, более значима у пациентов с тяжелым стенозом и сохранной функцией левого желудочка. Полученные данные могут помочь в реклассификации пациентов с умеренным аортальным стенозом, диагностика которого основывалась на традиционном усреднении последовательных ударов, в категорию пациентов с тяжелым стенозом. Заключение: важно учитывать вариативность градиента давления между сокращениями у пациентов с аортальным стенозом и сопутствующей фибрилляцией предсердий и соотносить ее со степенью тяжести стеноза и сократительными характеристиками.</p></abstract><trans-abstract xml:lang="en"><p>Purpose: To understand the significance of variation in gradient beat-to-beat pressure gradients in patients with significant aortic stenosis and concomitant atrial fibrillation and determine its relationship to severity.Methods: Assess the beat-to-beat variability of pressure gradients in patients significant aortic stenosis and concomitant atrial fibrillation from retrospective analysis of Doppler or catheter-based hemodynamic measurements and correlate with the preceding RR interval based on the corresponding electrocardiogram. Results: There is a direct correlation between the preceding RR interval and the mean pressure gradient in patients with significant aortic stenosis and concomitant atrial fibrillation. The variation seems to be more prominent in those with severe stenosis and preserved left ventricular function and may aid in classifying the degree of stenosis as severe in some with seemingly more moderate stenosis based on traditional averaging of consecutive beats.Conclusions: Variation in pressure gradients beat-to-beat are important to consider in patients with aortic stenosis and concomitant atrial fibrillation and correlate with severity and contractile characteristics.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>фибрилляция предсердий</kwd><kwd>стеноз аорты</kwd><kwd>эхокардиограмма</kwd></kwd-group><kwd-group xml:lang="en"><kwd>atrial fibrillation</kwd><kwd>aortic stenosis</kwd><kwd>echocardiogram</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Levy F., Rusinaru D., Marechaux S., Charles V., Peltier M., Tribouilloy C.
