Оценить 6-месячные результаты после реканализации хронических окклюзионных поражений коронарных артерий антеградным доступом.Методы. В исследование включено 117 пациентов, котором была выполнена реканализация хронических тотальных окклюзий (ХТО) за период с 2009 по 2012 годы. Через 6 месяцев все пациенты приглашались для проведения планового обследования, включающего проведение коронарографии, внутрисосудистого ультразвукового исследования и оптической когерентной томографии. Регистрировались клинические исходы, а также выполнялась оценка состояния коронарного кровотока, оценивалось развитие рестенозов, тромбозов и других негативных последствий.Результаты. Развитие летального исхода наблюдалось в 0,9 % от общего количества (n=1), острого инфаркта миокарда – в 0,9 % от общего количества (n=1), острого нарушения мозгового кровообращения – в 0,9 % от общего количества (n=1). Несостоятельность в зоне целевого поражения была выявлена у 23,1 % лиц; в 59,3 % случаев это сочеталась с наличием клиники стенокардии у данных пациентов. Основной причиной (у 81,5 % лиц) несостоятельности в зоне целевого поражения реканализированного сосуда являлось развитие внутристентового рестенозирования. Другими причинами, потребовавшими проведения повторных чрескожных коронарных вмешательств (ЧКВ) в зоне целевого поражения, являлись: развитие подострых и поздних тромбозов, поздней малаппозиции элементов каркаса стента и фрагментации стента.Для наблюдаемых пациентов характерной являлась высокая приверженность к постоянному приему в течение 6-месячного периода ацетилсалициловой кислоты (97,4 % от общего количества), клопидогрела (62,5 % от общего количества) и статинов (95,7 % от общего количества). Не наблюдалось взаимосвязи между прекращением приема антитромбоцитарных лекарственных средств и развитием подострых/поздних тромбозов в зоне выполненного чрескожного коронарного вмешательства.Заключение. Полученные данные свидетельствуют о хорошей сохранности результата через 6 месяцев после выполнения чрескожного коронарного вмешательства, что позволяет рекомендовать применение в клинической практике рентгеноэндоваскулярных методов для коррекции ХТО коронарных артерий.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess 6-month results after coronary artery chronic total occlusion (CTO) recanalization by antegrade approach.Methods. The study included 117 patients who had undergone coronary artery CTO recanalization for the period from 2009 to 2012. After 6 months, all patients were examined with coronary angiography, intravascular ultrasound and optical coherence tomography. Clinical outcomes, coronary patency, the development of restenosis, thrombosis and other negative events were registered.Results. Development of cardiac death was observed in 0.9 % of patients (n = 1), acute myocardial infarction – in 0.9 % (n = 1), stroke – in 0.9 % (n = 1). Target lesion failure (TLF) was detected in 23.1 % of patients, in 59.3 % of cases that was combined with the presence of angina symptoms. The main reason of TLF was restenosis development. It was observed in 81.5 % of patients. Other reasons requiring repeated PCI in the target lesion correction were subacute and late stent thrombosis, late malposition of stent structural elements and stent fragmentation.We observed high patients adherence to constant medical therapy including acetylsalicylic acid (97,4 % of adherence), clopidogrel (62,5 % of adherence) and statins (95.7 % of adherence). There was no correlation between the antiplatelet drugs discontinuation and the subacute/late stent thrombosis development.Conclusion.
