Благодаря ранней диагностике и новым методам лечения отмечено снижение смертности от злокачественных новообразований. Это привело к увеличению сведений о неблагоприятных сердечно-сосудистых последствиях самого противоопухолевого лечения. В ряде случаев попытки лечения отдаленных последствий химиолучевой терапии не всегда эффективны. Приоритетным становится раннее выявление нежелательных воздействий, их своевременная коррекция и проведение профилактических мероприятий. Ряд исследований показал, что нарушение диастолической функции предшествует снижению фракции выброса, однако до сих пор остается неясным роль ухудшения процессов релаксации в прогностической значимости развития ранней и поздней кардиотоксичности и сроках начала сердечно-сосудистой терапии.Методы71 женщина, из получивших лечение по поводу рака молочной железы (РМЖ), были распределены в группы в зависимости от наличия артериальной гипертензии (АГ) и кардиотропной терапии (КТТ): группа РМЖ – пациентки получали только противоопухолевое лечение (38 случаев), группа РМЖ+КТТ – пациентки без АГ и одновременно с лечением рака получали комбинацию карведилола и валсартана (22 случая); группа РМЖ+КТТ+АГ – пациентки с АГ, получающие кардиотропную терапию (11 случаев).РезультатыПосле окончания полного курса лечения рака молочной железы в группе РМЖ выявлено снижение соотношения трансмитральных потоков, увеличение массы миокарда и индекса массы левого желудочка, меридионального стресса, конечного диастолического давления в левом желудочке и конечно-диастолического напряжения стенки левого желудочка (р&lt;0,05).Было обнаружена прямая корреляция между степенью снижения фракции выброса левого желудочка и увеличением конечного диастолического напряжения стенки (r=0,34; р&lt;0,05), а также уменьшением соотношения трансмитрального потока и увеличением конечного диастолического напряжения стенки (r=0,41; р&lt;0,05).ЗаключениеНаиболее ранними изменениями структурно-функциональных параметров левого желудочка в результате противоопухолевого лечения является нарушение некоторых показателей диастолической функции. Назначение кардиотропной терапии снижает кардиотоксическое действие химиолучевой терапии.</p></abstract><trans-abstract xml:lang="en"><p>Methods71 female patients with breast cancer (BC) were divided into groups according to the presence of arterial hypertension (AH) and cardiotropic therapy (CTT): ВС group with antitumor treatment alone (38 cases), ВС+CTT group – patients with antitumor treatment who received carvedilol combined with valsartan and had no hypertension (22 cases); ВС+ CTT + AH group – patients who had hypertension and received carvedilol combined with valsartan (11 cases).ResultsAfter completing the full course of treatment for breast cancer in BC group a decrease in transmitral flow ratio, an increase in myocardial mass, left ventricular mass index, meridional stress, left ventricular end-diastolic pressure and left ventricular end-diastolic stress wall (p&lt;0,05) have been revealed.Direct correlation between the degree of reduction of ejection fraction and increase in the left ventricular end-diastolic stress wall (r=0,34; р&lt;0,05), as well as between decreased transmitral flow ratio and increased left ventricular end-diastolic stress wall (r=0,41; р&lt;0,05) have been found.ConclusionsThe earliest changes in the structural and functional parameters of the left ventricle as a result of antitumor treatment are represented by some parameters of diastolic dysfunction.The purpose of cardiotropic therapy is to reduce the cardiotoxic effect of chemoradiotherapy.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>профилактика кардиотоксичности</kwd><kwd>рак молочной железы</kwd><kwd>валсартан</kwd><kwd>карведилол</kwd><kwd>диастолическая функция</kwd></kwd-group><kwd-group xml:lang="en"><kwd>prevention of cardiotoxicity</kwd><kwd>breast cancer</kwd><kwd>valsartan</kwd><kwd>carvedilol</kwd><kwd>diastolic function</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Shuykova K.V., Emelina E.I., Gendlin G.E., Storojakov G.I.
