Ключевую роль в данной патологии занимает ишемическая болезнь сердца. Крайнее проявление ишемической болезни сердца (ИБС) – инфаркт миокарда является одной из основных причин осложнений и смертности пациентов с ИБС. В свою очередь, одно из наиболее грозных осложнений острого инфаркта миокарда (ОИМ) – разрыв сердца, который чаще всего приводит к смерти, при этом более 60% случаев приходится на догоспитальный этап. Многие исследования схожих групп пациентов показывают достаточно широкий разброс в оценке встречаемости и летальности данной патологии. Развитие разрывов миокарда имеют два пика частоты: первые сутки и 5–7 сутки от начала ОИМ. Наиболее часто встречается острый разрыв свободной стенки левого желудочка (ЛЖ) с обширным кровоизлиянием в перикард, приводящий к молниеносному летальному исходу. Развитие псевдоаневризмы левого желудочка весьма редко является исходом разрыва миокарда. Убедительных данных о частоте развития псевдоаневризм в результате ИМ нет (по данным многих авторов она составляет менее 0,5% от всех случаев ИМ). Именно этот тип разрывов является наиболее благоприятным с точки зрения возможности оказания помощи. Сложность оказания помощи пациентам с псевдоаневризмами левого желудочка состоит в их малой частоте встречаемости и зачастую бессимптомном характере течения, что осложняет диагностику данной патологии. Именно своевременная диагностика играет ключевую роль в возможности избежать фатального исхода, так как подавляющее большинство псевдоаневризм чрезвычайно нестабильны и, за исключением казуистических случаев, требуют экстренного или срочного хирургического вмешательства. В настоящей статье обсуждается клинический случай пациента перенесшего оперативное лечение по поводу псевдоаневризмы свободной стенки левого желудочка в результате инфаркта миокарда. Также в статье представлен краткий литературный обзор имеющихся разрозненных данных о факторах риска разрыва миокарда, методах диагностики и лечения данной патологии</p></abstract><trans-abstract xml:lang="en"><p>Today diseases of the cardiovascular system are the leading cause of death in many countries. The key role in this pathology is played by ischemic heart disease. An extreme manifestation of ischemic heart disease – myocardial infarction is one of the main causes of complications and mortality in patients with ischemic heart disease. One of the most formidable complications of acute myocardial infarction is heart rupture, which most often leads to death, more than 60% of cases occurring in the prehospital stage. Many studies of similar groups of patients show a wide spread in assessing the incidence and mortality of this pathology. The development of myocardial ruptures has two frequency peaks: the first day and 5–7 days from the onset of AMI. The most common case is acute rupture of the free wall of the left ventricle with extensive hemorrhage in the pericardium leading to a fulminant death.
